In the era of classical periodontology, success was considered as resolution of the disease including strategic extractions, the aim was to achieve PD reduction. The current definition of success in Periodontology is best expressed as “an effort towards preservation of natural dentition associated with patients’ well‐being” according to Pini Prato at all JCP 2019. New protocols in the management of the phase 1 for the treatment of periodontal disease can significantly improve the periodontal health and reducing the need for surgery. Also Periodontal surgery is deeply changed in the last few years because the aim is changed, and after the infection control the attention of the surgery is now on gain attachment level, and this is possible by means techniques that regenerate “attachment” rather then focus on bone or soft tissue only.
