Diagnóstico y plan de tratamiento para un paciente de clase II con mordida abierta. El paciente presenta una AFAI aumentada y mordida cruzada posterior, desvió de línea media y discrepancia posterior.
La discrepancia posterior provocó extrusión de las molares y como consecuencia hubo un cambio del plano oclusal.
La disminución de la altura vertical de los dientes posteriores implicaría una pérdida de soporte oclusal posterior, induciendo a una adaptación retrusiva de la mandíbula. En individuos en crecimiento la pérdida de soporte oclusal posterior causa compresión del cóndilo con la supresión subsiguiente del crecimiento condilar.
Las radiografías nos muestran cadenas musculares en parafunción y un patrón de crecimiento esquelético de clase II por influencia de la base de craneo.
El análisis del caso nos indica que al hacer expansión se provocará extrusión y aumentará la AFAI , acentuando la clase II.
